How to Support Your Cycle Naturally

A PCOS diagnosis often comes packaged with a warning about fertility — sometimes delivered almost as a foregone conclusion. “You may have trouble getting pregnant.” “We’ll deal with that when you’re ready.” For many women, this becomes a quiet source of anxiety that follows them for years before they’re even trying to conceive.

Here’s what we want you to know first: PCOS is a leading cause of anovulatory infertility, but it is also one of the most treatable. The same hormonal and metabolic imbalances that disrupt your cycle can, in many cases, be meaningfully addressed — and when ovulation is restored, pregnancy often follows.

This post is for women with PCOS who are thinking about conception, actively trying to conceive, or simply want to understand how their condition affects their reproductive health and what they can do about it.

Why PCOS Affects Fertility

Fertility depends on ovulation — the monthly release of a mature egg. In PCOS, ovulation is disrupted or absent in many cycles, not because the ovaries lack eggs, but because the hormonal signals that guide follicles through to maturity and release are dysregulated.

In a healthy cycle, the pituitary gland sends a coordinated pulse of follicle-stimulating hormone (FSH) and luteinizing hormone (LH) to the ovaries. FSH stimulates follicle development; a surge of LH triggers ovulation. In PCOS, this signalling is often skewed — LH tends to be chronically elevated relative to FSH, which stimulates androgen production rather than healthy follicle maturation. Follicles begin to develop but stall before ovulation, accumulating as the characteristic small cysts visible on ultrasound.

The underlying drivers of this disruption — most commonly insulin resistance, excess androgens, inflammation, or adrenal dysregulation — are what naturopathic treatment targets. When those drivers are addressed, the HPO axis can re-establish more normal signalling, ovulation becomes more consistent, and the fertile window becomes predictable.

Understanding Your Cycle First

Before jumping to fertility interventions, it’s worth understanding what your cycle is actually doing. This is harder with PCOS than with regular cycles, because the unpredictability of ovulation makes standard cycle tracking assumptions unreliable — but it’s not impossible, and the information is valuable.

Basal body temperature (BBT) charting tracks your resting morning temperature throughout the cycle. After ovulation, progesterone causes a sustained rise in BBT of approximately 0.2°C that persists until your next period. A clear, sustained temperature shift confirms that ovulation occurred. In PCOS, charts may show irregular patterns, delayed rises, or absent rises — all useful data that tells us what the cycle is (or isn’t) doing.

LH testing strips (OPKs) detect the LH surge that precedes ovulation by 24–36 hours. In PCOS, however, LH is often chronically elevated — which means OPKs can give false positives, or multiple surges without ovulation. They’re most useful when combined with BBT charting rather than used alone.

Cycle tracking apps can help organize your data but should be used cautiously with PCOS: apps that predict ovulation based on average cycle lengths don’t account for the variability of PCOS cycles and can give misleading timing information.

Progesterone testing at day 21 (or 7 days after suspected ovulation) can confirm whether ovulation occurred in a given cycle. A progesterone level above a certain threshold indicates ovulation happened; a low level suggests it didn’t. This is a straightforward blood test through Lifelabs or Dynacare and is a useful baseline when you’re starting to investigate fertility.

Understanding your ovulation pattern — even an irregular one — gives us important information and helps set realistic expectations as we work to support your cycle.

Naturopathic Support for Ovulation and Fertility in PCOS

Treating the Root Cause

The most effective fertility intervention for PCOS is also the most foundational: treating the underlying driver of the hormonal disruption. For insulin-resistant PCOS, this means improving insulin sensitivity through diet, movement, and targeted supplementation. For inflammatory PCOS, it means reducing the chronic inflammatory burden. For adrenal PCOS, it means supporting the stress response and HPA axis regulation.

When the underlying driver is meaningfully addressed, ovulation often begins to restore on its own — and the interventions described below can support and accelerate that process.

 

Nutrition for Fertility in PCOS

The nutritional principles for fertility in PCOS overlap substantially with those for managing insulin resistance. A whole food, low-glycaemic dietary pattern — rich in protein, healthy fats, fibre, and micronutrients — creates the hormonal environment most conducive to ovulation.

A few specific considerations for fertility:

Dietary fat quality matters for hormonal health. Cholesterol is the raw material from which sex hormones are made. Adequate intake of healthy fats — olive oil, avocado, egg yolks, fatty fish, full-fat dairy if tolerated — supports hormone production. Very low-fat diets can inadvertently compromise hormonal function.

Liver health supports hormone clearance. The liver is responsible for metabolizing and clearing excess estrogens and androgens. A diet rich in cruciferous vegetables (broccoli, cauliflower, Brussels sprouts, kale), adequate protein, and B vitamins supports healthy hormone metabolism. Minimizing alcohol during a pre-conception period is also worth considering, as alcohol directly impairs liver detoxification pathways.

Blood sugar stability throughout the day reduces the LH-driven androgen surges that interfere with follicle development. Regular meals with adequate protein and fat, avoiding long gaps between eating, and minimizing refined carbohydrates all contribute to a more stable hormonal environment.

 

Key Nutrients for PCOS and Fertility

Several nutrients play specific roles in ovarian function, egg quality, and cycle regularity that are particularly relevant in PCOS.

Myo-inositol has strong evidence not only for insulin sensitivity but for improving ovulation rates and egg quality in women with PCOS. Clinical trials have shown that myo-inositol supplementation increases the frequency of spontaneous ovulation, improves hormone profiles, and — in women undergoing assisted reproduction — improves response to ovarian stimulation. It is one of the most evidence-supported natural interventions for PCOS-related fertility.

Coenzyme Q10 (CoQ10) is an antioxidant that plays a central role in mitochondrial energy production — including within the mitochondria of developing eggs. Egg maturation is energetically demanding, and CoQ10 supports the quality of that process. Evidence suggests it may improve oocyte quality and ovarian response, and it is particularly relevant for women who are older or whose egg quality may be a concern alongside PCOS.

Folate (or methylfolate) is essential for DNA synthesis and cell division in the developing embryo, and is universally recommended pre-conception. For women with the MTHFR gene variant — which affects folate metabolism — the active form, methylfolate, is more bioavailable than folic acid. Given the prevalence of MTHFR variants, active folate is a reasonable choice for most women.

Zinc supports LH receptor function in the ovaries, healthy follicle development, and progesterone production in the luteal phase. It also has androgen-modulating effects that are relevant to PCOS specifically.

Vitamin D deficiency is common in PCOS and has been associated with reduced ovulation rates and poorer fertility outcomes. Testing and correcting vitamin D status is a simple, high-value step.

Iron deserves a mention specifically for women with PCOS who experience heavy or irregular bleeding. Iron deficiency is common in this group and can contribute to fatigue, poor egg quality, and impaired implantation. Iron studies should be part of any pre-conception workup.

 

Herbal Support

Vitex agnus-castus (chaste tree berry) is one of the most widely used herbs in naturopathic fertility care. It works by modulating dopamine receptors in the pituitary gland, which helps regulate LH secretion and support progesterone production in the luteal phase. In women with PCOS where LH is elevated and progesterone is low, Vitex can help restore a more balanced hormonal pattern and improve cycle regularity over time.

Vitex is not a quick fix — it typically takes two to three months of consistent use to see its full effect, and it works best as part of a comprehensive treatment plan rather than in isolation.

Tribulus terrestris has a traditional use in supporting ovulation and has some early clinical evidence showing improved ovulation rates in women with oligomenorrhea (infrequent periods). It’s used cautiously and in specific contexts, but may be considered alongside other interventions.

 

Movement and Stress

Physical activity improves insulin sensitivity and supports a hormonal environment more conducive to ovulation — but the intensity and type of exercise matters. Moderate-intensity movement, including resistance training and regular walking, is beneficial. Extremely high volumes of intense exercise, on the other hand, can suppress ovulation — a phenomenon more commonly seen in endurance athletes but worth being mindful of if you’re exercising intensively.

The psychological stress of trying to conceive is real and significant, and stress hormones do have measurable effects on ovulation. This isn’t about “just relaxing” — but finding sustainable ways to support your nervous system during this time, whether that’s through gentle movement, breathwork, adequate sleep, or community support, is a meaningful part of the picture.

When to Consider a Referral

Naturopathic care can be a powerful support for fertility in PCOS — but it works best within a collaborative healthcare framework, not in isolation from it.

We recommend that women with PCOS who are trying to conceive also be working with their GP or an OB/GYN, and we will always recommend a referral to a reproductive endocrinologist if:

  • You have been trying to conceive for 12 months without success (or 6 months if you are over 35)
  • There are known male factor fertility concerns
  • There is no evidence of ovulation despite treatment
  • There are other reproductive concerns (blocked tubes, uterine abnormalities, endometriosis) that may require investigation or intervention

 

Interventions like ovulation induction with letrozole or clomiphene, and assisted reproduction such as IUI or IVF, are highly effective for PCOS-related infertility and are sometimes the most appropriate path. Naturopathic care can work alongside these interventions to support your overall health, improve your response to treatment, and address the metabolic underpinnings that affect outcomes.

What a Realistic Timeline Looks Like

One of the most common questions we hear: how long will this take?

The honest answer depends on the severity of the underlying hormonal disruption, how consistently changes are implemented, and individual response. That said, for many women with PCOS who commit to comprehensive dietary and lifestyle changes alongside targeted supplementation, meaningful improvements in cycle regularity can begin to appear within two to four months. Ovulation may resume or become more consistent within three to six months.

We recommend beginning supportive treatment at least three to six months before actively trying to conceive, where possible — this allows time to restore cycle regularity, optimize nutrient status, support egg quality, and get a clearer picture of your fertile window before timing becomes a source of pressure.
