What Is Perimenopause?
Perimenopause is the transitional phase leading up to menopause — the period during which the ovaries gradually produce less estrogen and progesterone, and the menstrual cycle begins to shift and eventually cease. It is not a sudden event but a process, often spanning several years, during which hormone levels fluctuate widely and unpredictably before making their final descent.
The word “perimenopause” means “around menopause.” It ends — and menopause officially begins — when a woman has gone 12 consecutive months without a menstrual period. Everything before that 12-month mark, however erratic or symptomatic, is technically perimenopause.
It is one of the most significant hormonal transitions a woman experiences in her lifetime, and yet it remains one of the least prepared for and least supported.
When Does Perimenopause Start?
Perimenopause most commonly begins in the mid-to-late 40s, though it can start as early as the late 30s for some women. The average age of menopause in Canada is 51 — which means perimenopause often spans anywhere from two to ten years before that point.
There is no single trigger and no precise timeline. The transition begins when the ovaries start producing less consistent amounts of estrogen, disrupting the hormonal signalling that coordinates the menstrual cycle. This process unfolds gradually, and the pace varies considerably from one woman to the next.
Factors that can influence when perimenopause begins include:
- Genetics — the age your mother reached menopause is often a useful guide
- Smoking, which is associated with earlier onset
- Cancer treatments such as chemotherapy or radiation that affect ovarian function
- Surgical removal of the ovaries (surgical menopause), which causes an abrupt rather than gradual transition
- Chronic high stress, which can influence ovarian reserve over time
Perimenopause vs. Menopause: What's the Difference?
These terms are often used interchangeably, but they refer to distinct phases:
Perimenopause is the transitional period — typically several years long — during which hormones are fluctuating, symptoms may be significant, and periods are changing but have not yet stopped.
Menopause is a single point in time: the moment when 12 consecutive months have passed without a period. After this point, a woman is considered postmenopausal.
Confusingly, the most intense symptoms of the transition often occur during perimenopause, when hormone levels are fluctuating most dramatically — not necessarily at or after menopause. This is why many women are caught off guard: their symptoms begin years before menopause, at a time when they may not expect them, and standard bloodwork may not yet show clear menopausal patterns.
Signs and Symptoms of Perimenopause
Perimenopause affects virtually every system in the body, because estrogen and progesterone receptors are found throughout — in the brain, heart, bones, gut, skin, and more. This is why the symptom picture can feel surprisingly broad and sometimes difficult to attribute to a single cause.
Menstrual Changes
Changes to the menstrual cycle are often the first sign of perimenopause. Cycles may become shorter or longer, periods may arrive more or less frequently than before, and flow can vary significantly from month to month. Heavy or prolonged bleeding is common, particularly in early perimenopause, and can be significant enough to cause iron-deficiency anemia in some women.
Hot Flashes and Night Sweats
Hot flashes — sudden, intense waves of heat, often accompanied by flushing, sweating, and a rapid heartbeat — are the hallmark symptom of the perimenopausal transition. They occur because fluctuating estrogen affects the hypothalamus, the brain region that regulates body temperature, making it hypersensitive to small temperature changes. Night sweats are hot flashes that occur during sleep and can significantly disrupt sleep quality.
Hot flashes affect roughly 75% of women during perimenopause, though their severity and frequency vary widely.
Sleep Disturbances
Poor sleep is one of the most commonly reported and most impactful symptoms of perimenopause. Night sweats are one contributing factor, but even without them, falling and staying asleep becomes more difficult as progesterone — which has a natural sedative and calming effect — declines. Sleep architecture changes, with less time spent in deep, restorative sleep stages.

Mood Changes, Anxiety, and Irritability
Estrogen plays a significant role in the regulation of serotonin and dopamine — the neurotransmitters most closely associated with mood, motivation, and emotional resilience. As estrogen fluctuates in perimenopause, so does the stability of these neurotransmitter systems, contributing to mood swings, increased anxiety, low mood, and a reduced ability to manage stress that many women find deeply unfamiliar and distressing.
For some women, this is the first time in their lives they have experienced significant anxiety — and it can be easily misattributed to life circumstances rather than recognized as a hormonal shift.

Brain Fog and Memory Changes
Difficulty concentrating, losing words mid-sentence, forgetting names, and feeling mentally slower than usual are among the most alarming symptoms of perimenopause — and among the least discussed. Estrogen supports glucose metabolism in the brain and has neuroprotective effects; as levels fluctuate, cognitive function can be noticeably affected.
For most women, this cognitive fuzziness is temporary and improves as the hormonal transition completes. Understanding the mechanism behind it can provide meaningful reassurance.

Vaginal and Sexual Health Changes
Declining estrogen causes changes to vaginal tissue: the vaginal walls become thinner, drier, and less elastic — a condition known as genitourinary syndrome of menopause (GSM). This can lead to vaginal dryness, discomfort or pain during intercourse, increased susceptibility to urinary tract infections, and urinary urgency or frequency. Unlike hot flashes, which often improve with time, GSM tends to worsen progressively without treatment.
Libido may also decline, driven by hormonal changes, disrupted sleep, mood changes, and physical discomfort.

Weight Changes and Metabolic Shifts
Many women notice changes to their body composition during perimenopause — particularly an increase in abdominal fat — even without significant changes to their diet or activity levels. This reflects a real metabolic shift: declining estrogen affects how the body distributes and stores fat, and the decline in progesterone affects water retention and bloating. Insulin sensitivity also worsens as estrogen declines, making blood sugar regulation more challenging.
Skin, Hair, and Joint Changes
Estrogen supports collagen production in the skin, so its decline can lead to increased dryness, loss of elasticity, and more visible fine lines. Hair may thin or become drier in texture. Joint stiffness and achiness — often most noticeable in the morning — are reported by many perimenopausal women and reflect estrogen’s anti-inflammatory role in joint tissue.
Why Does Perimenopause Cause So Many Symptoms? The Hormonal Explanation
To understand why the perimenopausal transition can affect so much of how you feel, it helps to understand what estrogen and progesterone actually do — beyond their reproductive roles.
Estrogen is one of the body’s most wide-ranging hormones. It supports brain function, cardiovascular health, bone density, skin integrity, gut motility, immune regulation, and mood stability. When estrogen levels are stable, many of these systems hum along quietly in the background. When estrogen begins to fluctuate unpredictably — rising and falling erratically before its eventual decline — those systems are destabilized.
Progesterone, which declines earlier in perimenopause than estrogen, provides a counterbalancing, calming influence on the nervous system. It supports sleep, reduces anxiety, and offsets the proliferative effects of estrogen on the uterine lining. Its early decline — often before estrogen has significantly dropped — is responsible for many of the symptoms that characterize early perimenopause: worsening PMS, increased anxiety, disrupted sleep, and heavier periods.
The result of these two hormones declining at different rates, fluctuating unpredictably, and interacting with every major body system is the wide-ranging and highly individual symptom picture that perimenopause produces.
How Is Perimenopause Diagnosed?
This is where many women encounter frustration. Perimenopause is primarily a clinical diagnosis — meaning it is based on symptoms and history, not a single definitive blood test.
FSH (follicle-stimulating hormone) is commonly ordered to assess for menopause, but it fluctuates significantly in perimenopause and a single result is rarely conclusive. A normal FSH does not rule out perimenopause; an elevated FSH does not confirm it. Estradiol levels similarly fluctuate too much in early perimenopause to be reliably diagnostic on a single draw.
The most useful role of bloodwork in perimenopause is not to confirm the diagnosis but to rule out other causes of the symptoms (thyroid dysfunction, iron deficiency, and other hormonal conditions that can mimic perimenopause), to establish a baseline for monitoring, and to guide treatment decisions.

Resistance training preserves muscle mass and bone density, improves insulin sensitivity, supports cardiovascular health, and has meaningful benefits for mood and cognitive function. Weight-bearing aerobic activity — brisk walking, hiking, dancing — contributes to bone health and cardiovascular fitness.
The type of exercise matters in perimenopause: high-intensity exercise without adequate recovery can raise cortisol and worsen sleep and mood. Finding a balance of resistance training, moderate cardio, and recovery movement is key.

Sleep Support
Addressing sleep disruption in perimenopause requires a multi-pronged approach: managing the night sweats that interrupt sleep, supporting progesterone levels that influence sleep depth, and implementing sleep hygiene practices that protect sleep architecture. Frequently Asked Questions About Perimenopause
How do I know if I'm in perimenopause?
The most telling signs are changes to your menstrual cycle — in length, frequency, or flow — combined with symptoms such as hot flashes, disrupted sleep, mood changes, or brain fog, typically beginning in your 40s. Bloodwork can be useful to rule out other causes, but a normal hormone panel does not rule out perimenopause.
However, the first sign of perimenopause for some women are cognitive with symptoms of brain fog, fatigue, mood changes, etc. The documentary The M Factor 2 is worth a watch if this sounds like you.
Can perimenopause start in your 30s?
Yes. While the average onset is in the mid-40s, some women begin experiencing perimenopausal changes in their late 30s. If you are under 40 and experiencing these symptoms, it is worth investigating, as early perimenopause can have implications for bone and cardiovascular health that benefit from earlier intervention.
How long does perimenopause last?
On average, four to eight years — though the range is wide. Some women transition relatively quickly; others experience a decade or more of perimenopausal changes. The intensity of symptoms tends to peak in the two to three years immediately before the final menstrual period.
Is perimenopause the same as "the change"?
“The change” traditionally refers to the entire menopausal transition, which includes perimenopause, menopause (the final period), and postmenopause. Perimenopause is the active transitional phase of this process.
Can I still get pregnant during perimenopause?
Yes. Ovulation occurs irregularly during perimenopause, but it does occur — and pregnancy is possible until 12 full months have passed without a period. Contraception is still recommended for women who do not wish to conceive during this phase.
Will my symptoms improve on their own?
For many women, vasomotor symptoms such as hot flashes do gradually improve as the transition completes. Other symptoms — particularly genitourinary changes — tend not to resolve on their own and benefit from ongoing treatment. The metabolic changes associated with the hormonal transition (bone loss, cardiovascular risk, insulin resistance) also benefit from proactive management rather than a wait-and-see approach.
